Intervals:
21 color-coded questionnaires for use at 2, 4, 6, 8, 9, 10, 12, 14, 16, 18, 20, 22, 24, 27, 30, 33, 36, 42, 48, 54, and 60 months

Components:
21 color-coded questionnaires,
21 corresponding scoring sheets, the ASQ-3 User's Guide, the ASQ-3 Quick-Start Guide, online management (ASQ Pro, ASQ Enterprise, and ASQ Hub), and online questionnaire completion (ASQ Family Access)

Who completes it:
Parents/caregivers complete questionnaires; professionals, paraprofessionals, or clerical staff score them

Approximate time:
Each questionnaire takes 10–15 minutes to complete and 2–3 minutes to score

Validity and reliability:
More than 12,695 children have been screened and 18,232 questionnaires used to prove the accuracy of ASQ. Validity and reliability are excellent—validity is .82 to .88, test-retest reliability is .91, and inter-rater reliability is .92. Details in the technical report.
